Hm, I see a pattern. I've got a Radeon HD 4XXX as well. The card is powerful enough so it's not an issue of the video card itself. Maybe it's a problem specific to the xp driver on this card family. Are you running 800x600, and how does it behave in windowed mode compared to fullscreen?
