I always thought dc hackers used whispers. In any case not all dc hacks are created equal. I've seen some that seemed to have little to no effect used on me, one that required two characters and some time before it dc'd me, and one that dc'd me within a second or two of starting, and that's just the whisper ones.
As for defending against it, higher-end computer + lower-end internet connection might be enough to keep almost anyone from dcing you the traditional way since it relies on overloading your computer and not actually some sort of connectivity lag. I've heard various things such as ccing or going to the cash shop makes the dc hacker dc themselves. I'm not sure about that, but going to cs/mts would stop someone from dcing you.
