The expression...really hard to describe, comes off as a mixture of pain and...transformation? Maybe it just seems to be extended a bit too much. The lighting on the ear is a nice touch, but could be downplayed just a bit. But art is about the artist's view, and I am hardly one to criticize, so...

i think i did not explain very well what hyperacusis was, it is an extreme hypersensitivity to sound. hope k.o.r. does not mind that i have copy/pasted hyperacusis in her words here, so you can understand what this means:

Can only describe sound as torture [most sound,-level,pitch,frequency and how much noise have gone through that day also make a difference to it as well],the definition am use is deep painful shockwaves that roll back and forth throughout brain and body,crippling brain in a similar way to how some of own seizures do,the stronger sound gets-the more it mixes up different senses-will become visually impaired [from blurry/messy to completely VI] until recovered from overload,am also get temperary paralysis in both legs because the signals cut off when there's too much overload,should be getting that one a lot less when move to new residential as bedroom is professionally soundproofed.
just bring on old age and deafness.

did not know how else to convey other than with light, but kinda agree with you that the light was a bit overdone! it is supposed to be a light source rather than just a glow however. when i am overloaded one of my reactions includes scratching myself, hence the second glow with her hand - just in case you wondered.

still working on those expressions, so hard to do.
