The Evan's version of Final Attack does 150% at a 40% rate..
Is it 150% of your damage range or 150% of the skill used to trigger it? (Example I use Blaze, so its 260%*1.5)
And the proc chance, Blaze is more likely to proc it than Earthquake right? (3hits>1)
And is it possible to proc it twice in a Single Cast of Blaze? (since it hits thrice)

No and no. It activates based on skill used, not number of hits in a skill. If you use a skill, there's a 40% chance FA will activate. The number of times said skill hits is irrelevant.
