Where do we go to do it? I have already done the Silent Crusade through to completion... I hope that isn't what they mean by "upon completion of the quest to kill Coco." If so, is there a way to redo that quest?

You still can't get the card. The "card" you get when you kill Coco is the wrong item ID, so it's still impossible to get the Coco card.
